Hermes-Plugin unter /opt/hermes/plugins/decider-approval/. Ersetzt approvals.mode: smart durch Decider-Aufruf. Plugin ruft decider.py als Subprocess, bekommt command+context, gibt auto_approve/ask_user/abort zurück.
